Hawaii Manufacturing Assistance Program Update

The Hawaii Manufacturing Assistance Program (MAP) Grant Application Form is now posted online.

At this time applications may be submitted, but will not be processed until the final program rules are adopted. You will be notified once they are able to process applications.

As a reminder, HTDC CANNOT award state funds for grants until they receive all required information and documents from successful applicants. The required information and documents are:

1) Dun & Bradstreet DUNS number
2) Applicant must be categorized as a manufacturer as defined by the federal North American Industry Classification System (NAICS) codes: 31, 32, 33
3) Certificate of Good Standing (COGS) issued by the Hawaii Department of Commerce & Consumer Affairs (DCCA) Business Registration Division (BREG)
a. COGS serves as proof that applicant’s company is currently registered to do business in Hawaii. More information can be found at: http://cca.hawaii.gov/breg/
4) State of Hawaii – Department of Taxation Tax Clearance
a. Instructions for Form A-6 Tax Clearance Application can be found at: http://files.hawaii.gov/tax/forms/2014/a6ins.pdf
b. Form A-6 Tax Clearance Application can be found at:
5) State Vendor Code: To set this up, please provide:
a. A blank/voided invoice with company name and remittance address exactly as it should appear on the check payment
b. Please note that company information should be consistent on all documents (application, Certificates, etc.)
c. Federal Employer Identification Number (FEIN) or Federal Tax Identification Number or Social Security Number
6) Completed IRS Form W-9
